Health Screen Includes:

  • Lipid Glucose Blood Draw: Cholesterol, HDL Cholesterol, LDL Cholesterol, Glucose, Triglycerides, Total Cholesterol/HDL Ratio

  • Blood pressure, Pulse, Height, Weight and Body Mass Index calculation

  • Test results will be sent directly to you along with health education, all results are confidential

  • You will have the opportunity to get a Quadrivalent (4-virus protection) flu shot

  *For non-Anthem members, the cost for a health screening is $37 and the flu shot is $34 (cash, Visa, or Mastercard)

Employees enrolled in a CSU Anthem medical plan who complete the following activities by December 31, 2018 may be eligible to earn a $150 (taxable) incentive in 2019. Click here for additional information.

  1. CHECK-IN with a retirement counselor and complete this form by February 28, 2019

  2. PARTICIPATE in the CSU Health Fair to obtain biometrics and lipid/glucose panel

  3. ENROLL in Anthem's Healthy Lifestyles

  4. COMPLETE the Anthem Well-Being Assessment (must be completed between January 1, 2018 and December 31, 2018 to count towards the incentive in 2019)
