CHECK-IN + PARTICIPATE + ENROLL + COMPLETE

Employees enrolled in a CSU Anthem medical plan who complete the following activities may be eligible to earn a $150 (taxable) incentive in 2019. Click here for additional information.

  1. CHECK-IN with a retirement counselor and complete this form by February 28, 2019

  2. PARTICIPATE in the CSU Health Fair to obtain biometrics and lipid/glucose panel

  3. ENROLL in Anthem's Healthy Lifestyles by December 31, 2018

  4. COMPLETE the Anthem Well-Being Assessment
    (complete between January 1, 2018 and December 31, 2018 to count towards the incentive in 2019).

  OPTIONS FOR OFF-CAMPUS EMPLOYEES
 

Can't make the Health Fair? Don't worry, there are alternative options to help you earn the wellness incentive!

  1. Take the CSU Physician Form to your provider and submit by November 16, 2018 - or -

  2. Use the CSU Walk-In Facility Form at LabCorp
