Mid-Year Qualifying Events


You are permitted to make mid-year election changes within 30 days of an IRS approved qualifying event. Contact Human Resources for instructions on how to complete the requested election change. It is necessary to provide documentation to Human Resources to substantiate the qualifying event and to establish the eligibility for, and the effective date of, the requested change within 30 days of the qualifying event. Changes initiated or documentation received after 30 days will not be accepted.


Post-Separation Benefits (COBRA)


Under federal law (COBRA), Colorado State University is required to notify you of your right to continue coverage under "The Plan", which includes group health, dental, and/or a health flexible spending reimbursement account, when the coverage would otherwise end because of specific qualifying events described below. This notice is intended to inform you of your rights and obligations under the continuation provisions of the law.

Federal Regulations do not require employers to offer continuation of coverage to Domestic Partners or to the “qualifying” dependents of the domestic partner. Colorado State University has elected to extend COBRA benefits to Domestic Partners and their “qualifying” dependents.

Surviving Spouse Health Benefits


If you should die while employed by the University, your eligible dependent(s) who were enrolled at the time of your death, may be eligible for coverage continuation based on your appointment type.
